Nominations are being sought for the 2012 Achievement Award for Promoting the Health of the Public. Nominations must be received
by March 8, 2012.
The Dakota County Board of Commissioners established the Public Health Achievement Award in March 2000 to recognize and honor the contributions of Dakota County citizens who devote their time, energy and talents to create a healthy future for all citizens of the County. (Read about the

Awards recipients will be honored at the scheduled meeting of the
Dakota County Board of Commissioners on Tuesday, April 10, 2012 during the celebration of National Public Health Week.
Awards will be presented in three categories:
- Youth Award – an individual 17 years or younger at the time of nomination
- Individual Award – an individual 18 years or older at time of nomination
- Community Group Award – an organization, partnership or community coalition
Selection Criteria:
- Leadership - Nominee has demonstrated leadership abilities through work and service.
- Public Health Contributions - Nominee demonstrates originality and innovation in creating solutions to public health problems and has contributed to advancing improvements in the health status of the community through work and service.
- Collaboration - Nominee promotes collaboration and cooperation among individuals, organizations, and agencies.
- Advocacy - Nominee has been involved in preventing disease and disability, promoting health and healthy behaviors, and/or protecting the health and safety of county residents.
- Role Model - Nominee is respected by peers and is a positive role model for others.
- Evidence of Impact - The activities of the nominee have contributed to a successful outcome in terms of changes in the community.
To nominate an award candidate, please submit a nomination form by March 8.
- Individuals or groups may submit more than one nomination.
- Individuals or groups may nominate themselves.
- Nominations must address the selection criteria, and must be no longer than two typewritten, double-spaced pages.
