- Hampton
- Inver Grove Heights
- Lilydale
- Miesville
- Rosemount
- Vermillion
How to Apply
The fee for the license is $600. This is a two-year license. Please print and complete the following application form and Certification of Compliance with MN Worker's Compensation. Also enclose a copy of the educational materials applicant intends to use to educate employees on the sale of tobacco.
