Children and Family Services provides assistance to adolescents until they are 18 years of age. These services can be extended until they are 21 years old if the adolescent is attending high school or an equivalent educational program.
SELF Program
The SELF Program is a Transition program designed to work with adolescents who are 16 and older and are in permanent long-term placement settings. The focus of this program is to assure smooth transition into an appropriate adult program upon turning 18. Many times this is needed because most of these youth have varying degrees of disabilities and needs, or have no community supports upon leaving the child welfare system.
QUEST ProgramThe Quest Program is an independent living skills program for older adolescents who have experienced an out-of-home placement, and is sponsored by the SELF Program. The focus of this program is to help the adolescent prepare for independent living.
Transition TeamDakota County Social Services has established a Transition Team directed to facilitate effective transition from child to adult services and to identify needed resources. The Transition Team has representation from following Dakota County departments or divisions:
- Children and Family Services
- Adult Mental Health
- Adult Protection
- Developmental Disabilities
- Corrections
- Planning and the Supportive Housing Unit.
The Transition Team plans for youth exiting the child welfare system, homeless youth, and vulnerable youth. The Team identifies the needs of youth and young adults and initiates action to expand resources in the community. Services and resources will be tailored to meet the specific goals and needs of each youth/young adult.
