What services are available? The County can assist residents with an alcohol/drug abuse health assessment process (also known as Rule 25), which consists of:
- A chemical dependency/drug evaluation
- A determination of the need for chemical dependency/drug treatment
If the assessment recommends chemical dependency/drug treatment, funding for treatment, through the Consolidated Chemical Dependency Treatment Fund (CCDTF) may be available. CCDTF is a funding source of last resort, and other options for payment must first be explored.
How do I apply for services?
An application for services must be completed and can be done in one of two ways:

Visit our office during our business hours (Monday through Friday, 8:00 am - 4:30 pm) or |

Download and complete an eligibility application. Fax it to us at 651.554.6043 or email it to chcal@co.dakota.mn.us. In addition to your application, be sure to fax or email the verification/documentation requested. |
What documents do I need to provide with my application?
- Verification of residency (copy of your photo ID and a piece of mail with your name and current address from the last 30 days).
- Verification of income (attach the last two most recent proofs of income).

What happens once you have my application?
Once we receive the application, it will be reviewed and the individual will be given an authorization code along with a list of
Rule 25 Assessment Providers and he/she can call the Provider of their choice to set up an assessment.
