SAIL is a partnership of Dakota, Ramsey, and Scott Counties and the
Metropolitan Area Agency on Aging. Our mission is to reshape the long-term care system in the south east metro region by making more community options available and accessible to older persons. Funding is provided by a grant from the Minnesota Department of Human Services.
Projects include:
- Educating care managers, clients, and families on options that support seniors living in their own homes.
- Fostering the development of additional home care resources.
- Training for county and care system staff on early identification of dementia.
- Assisting cities to develop livable communities.
