Long-Term Care Consultations

A Long-Term Care Consultation helps people make decisions about long-term care needs. This service uses the expertise of social workers and public health nurses to help people stay in their homes by helping them find resources to make it possible.

How it works
When you contact Public Health Intake, they will schedule a time for your Long-Term Care Consultation. A social worker and/or public health nurse will come out to your home and meet with you to gather information about your health, how you are managing at home, and with what kind of activity you would like to be involved.
 
If you share financial information, the consultation will also inform you about your eligibility for public funding of services. The consultation usually takes about 2 to 2 ½ hours. During this consultation, you will receive information about the different long-term care options and available programs (both public and private) that can provide services.
 
The consultation could also include pre-admission screening to nursing homes, transition assistance to relocate people currently in nursing facilities and assessment and planning for home services.
