Community Health Assessment
A snapshot of health

​The Dakota County Public Health Department is requesting feedback from people who live or work in Dakota County on a new report, Healthy People/Healthy Communities: 2013 Dakota County Community Health Assessment.

The community health assessment provides a comprehensive picture of health in the county by updating residents on health issues that impact the quality of life and cause illness, injury and death in the county. It summarizes the top health concerns reported by residents and examines trends and issues that impact community health. Feedback will help the Public Health Department determine if the assessment has addressed key health topics and identified community concerns.

The community health assessment will be used by the Healthy Dakota Initiative to establish health priorities and identify strategies to improve health in the county. Guided by a steering group of area representatives from partner organizations including hospitals, clinics, schools and nonprofits, the Healthy Dakota Initiative assessment and improvement plans will be completed by fall 2014.
