Health Dakota Initiative
Help make Dakota County healthier

​As part of its comprehensive community health assessment and improvement project known as the “Healthy Dakota Initiative,” the Dakota County Public Health Department is seeking help from people who live or work in Dakota County.

To collect input from residents, the County developed a confidential online Community Health Opinion Survey that asks questions about varying topics such as access to medical and dental services, disease in the community, the health of babies and children, and environmental issues. No personal health questions are asked.

The survey is just one piece of the Healthy Dakota Initiative, which aims to engage community members in analyzing health-related data and developing strategies to address identified health concerns. Guided by a steering group of area representatives from key partner organizations including hospitals, clinics, schools and nonprofits, the Healthy Dakota Initiative assessment and improvement plans will be completed by fall 2014.
