A provider’s first license is a one-year license. Generally, child care licenses are renewed every two years. Three months prior to your license expiration date, you will receive a relicensing packet. In the packet, you will receive the following forms:
- Family Systems Licensing Application
- Certificate of Compliance – MN Workers’ Compensation Law
- Your Privacy Rights
- Family Child Care Licensing Checklist
- Enrollment Form
- Authorization for Background Study forms
- Training Log
You will need to complete these forms and submit them to the agency along with a fee of $200 for a two-year license. Submit the forms by mail to:

Your worker will make a drop-in visit to your home during the month mentioned in the letter with your relicensing packet. The renewal visit will be an unscheduled visit. Please inform your licensing worker of dates/times you are unavailable during your relicensing period.
At the visit, we will go through a short interview with you, check the paperwork you have on all the children in care, and make a thorough inspection of your home to see that it meets child care statutes and rules. We will answer any questions you have although please call your worker anytime with licensing questions and issues.
The following forms need to be available to show your licensing worker at the time of the visit along with all forms on the children in your care.
