First-time relicensing
A provider’s first license is a one-year license. When you have been licensed for nine months, you will receive a packet of forms in the mail to start the relicensing process. In the packet, you will receive the following forms:
  • Application
  • Bureau of Criminal Apprehension Forms
  • Home Safety Checklist
  • For Your Visit Form
  • Privacy Right Form
  • A letter stating the month of your home visit

    You will need to complete these forms and submit them to the agency along with a fee of $200 for a two-year license. Submit the forms by mail to:

    Your worker will make a drop-in visit to your home during the month mentioned in the letter with your relicensing packet. We do not tell you the actual date or time; however, if there is a time that does not work for you, we ask that you let us know.

    At the visit, we will go through a short interview with you, check the paperwork you have on all the children in care, and make a thorough inspection of your home to see that it meets the rules. We will answer any questions you have. If you need any resources, you may enter these on the For Your Visit Form, and we will try and accommodate your request.

    Basic relicensing
    The following forms are for currently licensed child care providers to complete and return to start the relicensing process.  Forms should be completed three months prior to expiration of the child care license.

    Child Care Application
    Child Care Checklist
    Enrollment Form
    Background Study Form (For each person over age 13)
    Privacy Rights Form (keep one copy for self and return 2nd copy to Dakota County)
    Training Record (copy showing your training and all other caregiver's and helper's training)

    Prepare for your visit
    The following forms need to be available to show your licensing worker at the time of the visit along with all forms on the children in your care.

    Escape Plan
    Provider Policies (your own or our form completed and shared with all clients)
    Mandated Reporter Notification Form (have a copy with each child's records signed by parent)
