Our services help seniors live in their communities as independently as possible. We work to make sure seniors and their families know what resources are available.
Access to our programs are determined by Medical Assistance or other financial eligibility. To find out if you're eligible for Medical Assistance, visit the
Minnesota Department of Human Services website. Contact us to determine your eligibility for programs the county offers.
There are many programs to help a person thrive in the community. Because each person is unique, we start with a MnCHOICES Assessment to determine if our programs are right for you. If they are, a case manager will identify your unique needs, coordinate services and provide ongoing support.
Getting started
To see if our services can help, you can request a MnCHOICES assessment. Find out more about a MnChoices assessment.
