Who needs to apply?
Uninsured residents can apply for the Behavioral Health Fund. Residents with private medical insurance can apply for the fund to be their secondary payer.
Residents using Medical Assistance or have an active PMAP do not need to apply.
Applying for Behavioral Health Fund via direct access
To apply for an assessment, applications can be done in one of three ways:
- Download and complete an
eligibility application. Fax it to Chemical Health Intake at 651-554-6043 or email it to chcal@co.dakota.mn.us. - Complete an application with your community provider at the time of assessment. Find a provider with the
Minnesota Department of Human Services Fast Tracker or the
Dakota County list of providers.
- Visit the Northern Service Center during our business hours (Monday through Friday, 8 a.m.–4 p.m.). The office is located at 1 Mendota Road West, Suite 300, 3rd Floor, West St. Paul, MN 55118. County staff is available for assistance, but the process is more efficient if you work directly with a provider.

Eligibility Application
- Verification of residency: Copy of your photo ID and a piece of mail with your name and current address from the last 30 days. (No proof required for people experiencing homelessness. Only city of residence needs to be provided.)
- Verification of income: Attach the last two most recent proofs of income, such as pay stubs or bank statements. (No proof required for people unemployed.)
No proof is needed for anyone held in incarceration.

After you apply
Once Dakota County receives the application, it will be reviewed. Once approved, a 6-month eligibility span will start to access chemical health services. You will need reapply every 6 months unless you Medical Assistance or PMAP become active.
