You can purchase a certified copy of a death record for anyone who died in Minnesota from 1997 to present. Prior to 1997, records are available for those who lived or died in Dakota County.
Death record books from 1870 through 1945 are available for viewing at the Administration Center.
For non-Dakota County death records prior to 1997, contact the
Minnesota Department of Health. Additional information can be found at the
Minnesota Historical Society's website.

Purchasing in person
A person must be an
authorized requestor as defined by the Minnesota Department of Health by filling out a
Certified Death Certificate Application.
Bring the unsigned form and fees to one of six locations listed above.
Show proof of identity such as a valid driver’s license or passport. A deputy registrar will witness your signature.
Purchasing by mail
A person must be an
authorized requestor as defined by the Minnesota Department of Health by filling out a
Certified Death Certificate Application by Mail.
You must sign the completed form in front of a notary — the notary’s signature and stamp must be on the application.
