Current providers
Providers of Home and Community Based Services (HCBS) or Minnesota Health Care Programs (MHCP) should complete the HCBS Provider Form.
This form is for current service providers that serve Dakota County residents. Fill out of the form if your organization would like to share information, including openings and referral process. This form is for information purposes only and is not used for provider licensing or authorization.

The state enrolls new providers. County contracts are not needed for most services funded though Medical Assistance (MA). For information on provider enrollment, please visit the Minnesota Department of Human Services (DHS) provider enrollment page.
Some services need licenses. For more information on licensing, visit the state licensing page.
Lead Agency Assurance Statement
DHS uses the Lead Agency Assurance Statement (DHS-6383) form to verify providers meet qualifications to provide some services. Find the
Lead Agency Assurance Statement (DHS-6383).
- Section 1: For Caregiving Living Expenses, contact the service recipient's case manager. For everything else, contact Dakota County's contracts team at cscontracts@co.dakota.mn.us. These services need a contract with the county.
- Sections 2 and 3: Dakota County does not manage these services. Please enroll directly with DHS.
HCBS Day Services
HCBS Day Services are for adults (ages 18–64) with disabilities. These include:
- Day support services
- Day training and habilitation
- Prevocational services
Dakota County has to follow a special process called the Needs Determination Process, which is set by DHS. If you need a needs determination, or to learn more, please fill out the Dakota County Needs Assessment Request form.
Adult Day Centers no longer need to complete a needs determination. Adult Day Center development will need to follow DHS processes. For more information, see the CBSM - Adult day services page.
