to schedule a time for an assessment. A social worker and/or public health nurse will come to your home to gather information about your health and how you are managing at home. The visit usually takes about two hours. You will receive information about long-term care options and programs. There is no charge for the visit.
These resources can also give you information on services for the aging and disabled.
