Social Services continues to provide services through our various access points. For many programs, we are temporarily changing how we deliver critical community services to help reduce the spread of COVID-19. Social Services will to continue to provide essential services while also protecting the well-being of our staff, contracted partners and all County residents.

The Community Living Services unit can help you get started on finding solutions that will help you or a loved one live as independently as possible.
Getting an assessment
Contact Community Living Services by email at firstname.lastname@example.org or phone at
651-554-6336 to schedule a time for an assessment. A social worker and/or public health nurse will come to your home to gather information about your health and how you are managing at home. The visit usually takes about two hours. You will receive information about long-term care options and programs. There is no charge for the visit.
