Providers of home and community based waiver services (HCBS)
Providers interested in serving people receiving HCBS/waiver services should contact the Minnesota Department of Human Services to learn more about licensure requirements, provider enrollment and service standards.
Providers interested in HCBS day services
Day services are a group of Home and Community Based Services (HCBS) for adults 18-64 with disabilities that include adult day, day support services, day training and habilitation, and prevocational services. In order to establish these services, Dakota County must follow a Needs Determination Process that was established by the Minnesota Department of Human Services.
Dakota County is not accepting applications from prospective providers of facility-based day services at this time.
