Providers of home and community based waiver services (HCBS)
Providers interested in serving people receiving HCBS/waiver services should contact the Minnesota Department of Human Services to learn more about licensure requirements, provider enrollment and service standards.
Providers interested in Housing Support
Upon submission of the Vendor Interest Form, interested providers will be asked to complete an application for Housing Support (formerly Group Residential Housing).
