Become a new County vendor
Suppliers and potential suppliers to Dakota County must complete and return the following:

If applicable, you will also need to provide a Certificate of Insurance.

Certificate of Exemption Form ST3

Contractor Affidavit
State Law requires all final invoices related to construction projects include a Contractor Affidavit certifying the contractor and any subcontractor have fulfilled the requirements of Minnesota withholding tax laws in order to receive final payment.  FactSheet13.pdfSee Fact Sheet 13 for additional information.
