The Dakota County Community Health Improvement Plan (CHIP) is a key step in a long-term, systematic effort to address public health issues identified in the community health assessment.
Community action teams developed goals, measurable objectives and action steps for the one priority planning area identified by the Healthy Dakota Initiative: improving adult mental health and reducing adolescent suicidal thoughts.
The plan is intended to be a call to action and a guide for community stakeholders to improving health in Dakota County.
