Community Support Programs

Social Services contracts with two mental health providers who offer client outreach, medication monitoring, crisis assistance, assistance with independent living skills, development of employability and work opportunity services, socialization opportunities, housing support, and benefits assistance. The providers have center-based services in West St. Paul and Eagan and offer outreach services throughout the county. These providers may be accessed directly without a referral:
