Before you enter the Ages and Stages Questionnaire (ASQ) website to begin the process of enrolling in the Dakota County Follow Along Program online, please review the following:
- You are leaving Dakota County's website and are going to Brookes Publishing Co. Inc.'s (“Brookes") Ages & Stages Questionnaire (“ASQ") website, which is not operated by Dakota County. Dakota County is not responsible for the content or availability of the ASQ Website.
- By entering information into the ASQ website you are agreeing to be bound by the Brookes End User License Agreement, Terms of Use and Privacy Policy, which are accessible from the login page of
www.asqonline.com.
- According to the Brookes Privacy Policy, as of Jan. 10, 2019, authors or other individuals associated with Brookes may use information “stripped of all personally identifying information" for research purposes. Dakota County does not have the ability to review or control how Brookes de-identifies your data for this purpose. Any time you enter information in a third party website, that information may become public. If you have questions about this process or the uses of the data, you should contact Brookes before you decide to use the ASQ website, or you may request a paper version of the ASQ, which means that you will not need to enter any information into the ASQ website.
- After you click the link below please review the Tennessen Warning before you enter data into the ASQ website.
- Dakota County Public Health will have access to the data you enter into the ASQ website. However, Dakota County's access to data located on the ASQ website is subject to the Brookes End User License Agreement, Terms of Use, and Privacy Policy.
- After you enter data into the ASQ website, Dakota County Public Health may transfer the data to a Dakota County network location and delete the data from the ASQ website.
- You have the option of not using the ASQ website and to answer the questionnaire and provide data in paper format. If you prefer this option, please contact Dakota County by calling 651-554-6178. Selecting the paper option will not in any way affect you or your child(ren)'s eligibility to participate in the Follow Along Program or receive other services from Dakota County Public Health.
