A case manager is a professional assigned to help you determine the services and supports that will help you to live a safe and healthy life.
Case managers respond to your needs regarding your physical and mental health. They follow state and federal rules that govern services.
An assessment will determine if you qualify for case management.
After a MnCHOICES Assessment, the county will assign you a case manager. Request a MnCHOICES assessment to see if you are eligible for services.
Assigning a case manager
- You will be paired with a case manager after you have completed an assessment and have been determined eligible.
- Dakota County contracts with and supports multiple agencies to provide waiver and Rule 185 case management services.
- If more than one sibling in a family is being served, you can request the same case manager for both persons.
Case management expectations
You can expect your case manager to:
- Plan face-to-face visits with you.
- Provide service planning, system navigation and advocacy that is:
- Based on assessed needs
- Person-centered
- Review all informal and formal supports for you.
- Make referrals for services, but it is your choice to accept and to choose the provider.
- Complete ongoing assessment of changing needs.
- Partner with schools, medical providers, therapists, behavior services, employment providers and other services provided through medical assistance or waivers.
