Whenever you are asked to give us private or confidential information about yourself, you must be told:
- The purpose and intended use of the information by this agency.
- If you are legally required to give us the information.
- What will or might happen if you give us the information, and what will or might happen if you do not give us the information.
- The identity of other persons or agencies who may legally see the information.
