The Consumer Support Grant (CSG) is based off of eligibility for Personal Care Assistance Services and provides monthly cash grants to children and adults with certified disabilities living at home. The grant is an alternative for individuals and families who are eligible for the state plan MA Home Care programs of: Personal Care Attendant (PCA) Services, Home Health Aide (HHA) Services, and/or Private Duty Nursing (PDN).

Prior to beginning the Consumer Support Grant process, a guardian/designee must attend an orientation and submit an approvable Consumer Support Grant plan.

Developing a plan

How grants are determined
The state assigns a monthly grant amount based on the need for assistance in daily living activities, complex health needs and behavioral challenges. These are evaluated during a MnCHOICES Assessment completed by a Certified Assessor.
Based on the results of the assessment, families can choose between Personal Care Assistance hours or the Consumer Support Grant.
What funds can be used for
The Consumer Support Grant can be used for support or services for the disabled family member that are not covered by Medical Assistance or insurance.
