The Consumer Support Grant (CSG) is a state-funded alternative to Medicaid home care services of Personal Care Attendant (PCA) Services, Home Health Aide (HHA) Services, and/or Private Duty Nursing (PDN).
Medicaid services are paid for with a combination of state and federal funds. CSG allows eligible participants to convert the state portions of payments for specific home care services into a cash grant. The monthly CSG grant replaces fee-for-service home care services payments.
The Consumer Support Grant gives participants more control and flexibility in how they choose their services.
Eligibility requirements
To be eligible, a person must meet all these criteria:
- Are able to direct and purchase their own care and supports or have a family member, legal representative or other authorized representative available to purchase, arrange and direct care on their behalf
- Be enrolled in or eligible for Medical Assistance (MA)
- Be eligible to receive home care services from MA Home Care Program such as Personal Care Assistance (PCA), Home Health Aide (HHA) or Private Duty Nursing (PDN)
- Demonstrate limitations in everyday functioning, such as self-care, language, learning, mobility, self-direction, capacity for independent living and economic self-sufficiency
- Require on-going support to live in the community
- Live in a natural home setting
Eligibility for CSG is determined by a MnCHOICES assessment, to determine if an individual is eligible for PCA, HHA or PDN services. To request a MnCHOICES assessment, please call Community Living Services (CLS) intake at 651-554-6336.

Developing a plan
Participants must develop an approvable CSG plan that identifies the support, goods and services needed. Participants may hire a support planner to assist with plan development. The CSG plan must be submitted to the assigned case manager or CSG Coordinator for approval before receiving a grant.
How grants are determined
Grant amounts are based on home care assessments and ratings for Home Health Aide, Private Duty Nursing or Personal Care Assistant services. Generally, the amount of the CSG grant is determined for each person using the results of their MnCHOICES or PCA assessment.
What funds can be used for
The Consumer Support Grant can be used for support, services and goods needed for the person to live in their own home. Support, services and goods must be related to the person’s functional limitations. CSG can cover costs that are over and above the typical cost of caring for a person.
